What to Bring


A successful visit starts with being prepared. Make sure you have the following items ready:


  • Driver's License: This essential item verifies your identity, ensuring both your safety and privacy.


  • Insurance Card(s): Keep these documents ready to simplify the paperwork. This can help us quickly check your benefits and reduce your waiting time


  • Prescription from Your Physician (if applicable): If you have a perscription from your doctor for physical therapy, bring it. However, it is not necessary.


  • New Patient Information Sheet: Download and complete this form before your visit. It provides vital information regarding your medical history and current health concerns, which will expedite the care we can offer.

Dress Comfortably


Comfort is key during your visit! We recommend wearing loose, comfortable clothing and suitable shoes for exercising. This not only makes you feel at home but also enhances our ability to assist you.
